Individuals receiving certain suspensions, cancellations and revocations of their driving privileges are entitled to an Administrative Review. You may use the form below to request a review only if the notice that you received from DMV states that you have the right to request an Administrative Review.
Please Note:  Do not use this form to request a review if you are eligible for, and want to request a hearing.
An Administrative Review is not a face-to-face meeting between you and a DMV representative. DMV will review the documents that led to your suspension, revocation or cancellation and the action taken. Unless an error was made, DMV’s action will be upheld. Furthermore, DMV will not rescind the action pending the outcome of an Administrative Review.
Please Note: If you are considering an Administrative Review because you believe a court or other authority wrongly convicted you, the proper avenue for appeal is with them. We can only determine if the right information was entered on the appropriate DMV record and if the applicable laws and rules were followed by DMV, not whether the court or other authority made the correct determination.
DMV generally responds to Administrative Review requests within two weeks.
